Current State Requirements

Current state requirements include:
  • Certificate of Fictitious Name: This form must be filed with the Churchill County Clerk/Treasurer (NRS 602) for all businesses operating under a name that is in any way different from the name(s) of the owner(s) or the name of the entity registered with the Secretary of State.
  • Child Support Statement: This form is required of all owners, but not required for registered entities.
  • State Business License: The application is submitted through the Nevada Secretary of State’s office. We will need a copy of the state business license or exemption.
  • Nevada State Sales / Use Tax Permit: Registration is required with the Department of Taxation in order to obtain a taxation permit or exemption statement.
  • Industrial Insurance (workers’ compensation) coverage: You must provide a certificate of insurance or complete the D-25 form to claim an exemption as a business with no employees.
  • Registered Entities: A copy of the certificate in good standing issued by the Nevada Secretary of State is required.
  • State Health Permit: This is required for all businesses handling food, beverages, or cosmetics and must be obtained from the Nevada Bureau of Health Protection Services; contact them at (775) 423-2281 or (775) 684-4200. 
 
You will also need to provide a copy of any other permits or licenses required for the type of business (i.e., Department of Motor Vehicles, Nevada Department of Transportation, real estate broker, massage therapist, Board of Law, mortgage lending, veterinary medicine, contractor's license, etc.).
